Dear All,

I have sent a couple of pictures of the jack (front and back) to Martin 
Spaink, who has graciously volunteered to place them on a photo site 
which should be accessible to everyone. I am not sure the pictures 
reveal as much detail as I'd like but may reveal the identity of the jack.
